I understand and accept the policies on this website and agree to PAY MY ACCOUNT IN FULL when due. I also agree to pay any late fees ($25/mo) and all fees associated with the collection of any outstanding balances on my account. I understand that by completing this registration form and enrolling my son/daughter in PPL Level classes I am agreeing to the terms and conditions as stated in the Pre-Professional Program Contract and Company Addendum. I hereby release photo and video footage for Stoughton Center for the Arts, Inc. publicity.

I, as parent/guardian of my child attending in-person classes at the Stoughton Center for the Arts, Inc., knowingly and willfully consent myself and my child to the following information below:
I confirm that my son/daughter will not attend in person classes if they are showing any of the following symptoms of COVID-19: fever/chills, shortness of breath or difficulty breathing, loss of sense of taste or smell, dry cough or sore throat.
I confirm that if my son/daughter, myself, or any member of my household tests positive for COVID-19 I will alert the Director immediately.
By attending in person classes you acknowledge that an inherent risk of exposure to COVID-19 exists in any public place where people are present and you assume all risks related to exposure. You will not hold Stoughton Center for the Arts liable for such exposure or the potential of such exposure.
