Medical Consent(Required)

I authorize medical consent as stated below.

In the event I cannot be contacted through reasonable efforts, I, the parent/guardian of the student on this form, hereby empower and grant to the Stoughton Center for the Arts, Inc. (namely Natalie Norlin) permission to consent to and authorize medical treatment for the individual named above. This authorization shall be for the current Season at any function I am not attending. I do hereby indemnify and hold harmless the health care providers and entities and other persons who act in reliance upon this authorization.

Payment Agreement and Liability Waiver(Required)

By checking the box below, you agree to the following: I understand that there are certain risks involved with participating in any activity. I expressly understand and agree that neither the Stoughton Center for the Arts, Inc. nor any of its employees will be held responsible or made subject to any claim, other than for willful or reckless conduct directly attributable to them, seeking to assess damages or liability for or arising from personal injury or property damage to myself or other person in whose behalf this form is now signed as a result of actual or proposed participation in classes. I understand and accept the policies on this website and agree to PAY MY ACCOUNT IN FULL when due. I also agree to pay any late fees ($25/mo) and all fees associated with the collection of any outstanding balances on my account. I understand that by completing this registration form and enrolling my son/daughter in PPL Level classes I am agreeing to the terms and conditions as stated in the Pre-Professional Program Contract and Company Addendum. I hereby release photo and video footage for Stoughton Center for the Arts, Inc. publicity.

Registration Fee and Payment

I understand that a $25 registration fee will be invoiced and processed with August tuition.
